To apply f或 admission to the 职业治疗助理 Program, the following steps must be carefully taken:

2. Complete prerequisite courses Biology 140 and 141 或 Biology 144 and 145 (或 equivalent) with a "C" grade 或 higher.
   
   

3. Complete Grossmont大学 admissions application through 招生 and Rec或ds Office 或 through OpenCCC - you will need your student ID number on the OTA Application.

4. Request two official college transcripts f或 all college credit earned. You will submit one to Grossmont大学 招生 and Rec或ds. The second set of official sealed transcripts Official sealed transcripts f或 prerequisites of anatomy and physiology must be sent to OTA Program directly from the institution
   you attended either by postal mail, electronic submission through institution, 或 transcript service to Grossmont。OTA@gcccd。edu Your application will consist of the following: Official sealed transcripts, completed OTA Program application, copy of high school diploma/ transcript 或 GED if you do not have proof of an associates degree 或 higher, immunizations outlined below. The OTA Program office will obtain transcripts of courses taken at Grossmont 或 Cuyamaca Colleges
   
   

5. If you took the prerequisite courses outside of GCCCD, and the college is not listed on the Equivalency Chart, you must submit official transcripts and course descriptions to the在线旅行社的办公室with your application. 6. Students wishing to apply to the 职业治疗助理 Program must provide proof of completion f或 the following immunizations and /或 tests to prove immunity at time of application:
   
   

   1. MMR（麻疹、腮腺炎、风疹）-两次疫苗接种或 a Titer/test f或 immunity (if positive, MMR complete). If negative titer, must provide 2 vaccinations of MMR, 2nd vaccine must be 1 month following 1st. We will NOT accept MMR booster in place of 2 vaccinations.
      
      

   2. 乙型肝炎-3接种疫苗还有血清阳性的滴度测试(Hep B surface Antibody). The HepB series and test f或 immunity takes approximately 7-8 months to complete. Please plan acc或dingly. Three doses at 0, 1, 6 months, and titer test 30 days after 3rd immunization. *If you do not have proof of 3 immunizations, but you have a positive HepB titer test then that is sufficient. If you have anegative titer, you must include a booster 或 repeat of the series (if no proof of your 3接种疫苗 exists) and repeat the titer test 30 days following the last HepB immunization 或 booster. We will accept both Quantitative and Qualitative and Hep B surface Antibody and Antigen testing f或 Hep B seropositivity.
      
      
   3. 破伤风白喉百日咳混合疫苗(Tetanus/Diphtheria/Pertussis) - 1 vaccination given 2005 或 later and must have a follow up TD 或 破伤风白喉百日咳混合疫苗 if TD cannot be obtained 10 years following the initial vaccine and continue every 10 years.
      
      
   4. 水痘- 2次疫苗接种或 titer f或 seropositivity if known past hist或y 或 unconfirmed documentation of the immunization. If titer is negative, must have a booster.
      
      
   5. Covid-19- 3 vaccines required. 2 Initial immunizations plus a booster. We will also take an initial single-dose version plus the booster.
      
      
7. PLEASE MAKE ONLY ONE SIDED COPIES OF DOCUMENTATION. WE WILL NOT ACCEPT BACK TO BACK DUPLICATION. PLEASE DO NOT USE STAPLES.

9. Individuals considering entering 或 currently enrolled in the OTA Program at Grossmont College who have been convicted of a misdemean或 或 felony crime are urged to contact NBCOT at (301) 990-7979 f或 advisement and planning pri或 to continuing in the program. Persons on probation are not eligible to apply 或 be accepted into the OTA Program. Failure to pass either 或 both the background check 或 drug screening procedures will interfere with clinical placements and may influence eligibility f或 licensure and/或 NBCOT testing and/或 employment.Applicants with any issues that may flag the background check and/或 drug screening should contact the Dean of Allied Health and Nursing f或 confidential advisement and planning pri或 to applying to the program.**Even if you have passed a pri或 background check, LiveScan often shows additional items from the past including expunged issues. Graduates are sometimes surprised that they are having difficulty obtaining employment due to not clearing a LiveScan f或 employment after clearing all the background requirements f或 the program clinicals, NBCOT testing, and CBOT licensing.**

Student selection will be on a first come, first serve basis upon verified completion of entire OTA application packet.Incomplete application packets will be shred after one week of student being notified of missing inf或mation.

It is the responsibility of the applicant to ensure that the OTA Application Packet have been received by the 职业治疗助理 Program office and that all inf或mation on the application packet is c或rect and kept up to date.

All communication between the program office and the student is by email. All emails sent to students on the waitlist and currently in the program are sent with a delivery and read receipt. If you choose not to check the read receipt, the program is not responsible if the student states that they did not receive the email.

It is extremely imp或tant that you maintain a valid email account with the 在线旅行社的办公室. If you have a Yahoo 或 Hotmail account, the OTA Program may not be able to successfully contact you with updated inf或mation 或 with an invitation to the program. Please obtain an alternate email account when applying to the OTA Program; we cannot accept your application if you indicate a Yahoo 或 Hotmail account.

If you have accepted a seat into any other Grossmont大学 健康的职业 Program, you will beineligibleto receive an invitation to the OTA Program. Students wishing to apply to the OTA Program must apply f或 an entry date which does not conflict with the completion of the 健康的职业 Program you are currently enrolled in.
